2. Please be sure to include your phone and email address with your check.
3. Then please tell us a little about yourself and your practice, using the NEW MEMBER INFORMATION, below.
4. In a few days we will get back to you with information on how to add your listing to our on-line Directory and instructions for joining in on our tele-conference calls.
